Referring a Patient

There are three Paediatric Palliative Care Teams in NSW. This page includes information on how to make a referral or obtain further information about the referral process.

As tertiary hospitals, all children who require Specialist Paediatric Palliative Care (PPC) input require a medical referral.

If you feel a referral to palliative care is appropriate for a patient, please suggest that the family contact their GP, Paediatrician or treating team to discuss/organise a medical referral.

To make a referral to the Paediatric Palliative Care team please complete the referral form and mail /email or fax to the appropriate team. A phone call to the relevant service assists in ensuring a timely response to the referral.

If you are unsure about if a child the suitability for referral, or which specialist service is the most appropriate, any of the services can be contacted for advice.

When referring a child to the Palliative Care team, please confirm that the family have been notified about the referral.
